Student-Athlete Medical Checklist
Dear Student-Athlete:
 
Welcome to Concordia University Athletics! Please read this letter carefully, print and complete the required forms, and return them. Please note that student athletes will not be able to participate until all forms have been received and approved by the Athletic Training Staff. Copies of the informational letters and forms can be found on our website at www.gocugo.com/medicalformsAll completed forms must be received by July 15, 2011.
 
If you are a new student-athlete (freshman or transfer) at Concordia this year, you will need to undergo a pre-participation physical examination in order to be cleared to participate in team practices or events.  Concordia’s team physicians will be conducting physicals on campus at a cost to you of $40. Please report to the CU Athletic Treatment Center at the times listed below for your respective team.

will undergo a medical update that will be conducted by the Concordia University Athletic Training staff at no cost to you. Please note that as a returning student-athlete, this update will be in place of your physical examination.

ALL student-athletes are required to be enrolled in a primary insurance plan within the United States that covers injuries resulting from participation in intercollegiate sports. To ensure the proper information is recorded, please enclose a copy of your insurance card front and back. 

It is important that you accurately complete and sign the Insurance letter, HIPPAA form, Medical History, and  pre-participation form.  If you have had any surgeries in the past six months, please enclose a copy of the operation report, which may be obtained from your surgeon. Also, please note that the surgeon will need to provide authorization – in writing - for your participation. If you are recovering from an operation or injury and have a formal rehabilitation program from a therapist or physician, please enclose a copy of that report so that we will be able to continue your regimen here at Concordia.
