4. Make a copy of your medical insurance card and include with items A, B & C.
5. Physical Form: We understand that you have an opportunity to undergo a physical examination with your personal physician, however, we strongly recommend that all of our student-athletes wait to do it with CU team physicians. In the event that you select your physician, plaease contact us as all physicals are required to be completed with the CU physical examination form. Thank you.
